Choose the correct version, download it, fill out the information regarding ministers, readings, prayers, and music, save a copy locally, and return via e-mail to Mary Ella Wielgos, Pastoral Associate for Worship and Sacraments at mwielgos@saint-max.org. 

The planning sheet is due 3 months prior to your wedding date.  

To complete these liturgy planning sheets, you will need to consider the archdiocesan book Celebrating Marriage (normally given at the "Before We Say 'I Do'" workshop) as well as the musical choices below.
